If you find yourself near Lanka Estate, on the trail above the southern river bank, you can chance upon some ghouls attacking a man and his cart.

Help the trapped man.
Kill the ghouls, then approach and talk to the man about his cart that is loaded with corpses.
Say whatever you want to him, but insist that you must burn the cart and any plague victims that might be nearby. Gert will let you torch the cart, albeit with some reluctance.

Burn the cart with your Igni skill and continue on your way.
Some days later you might run into Gert again on the battlefields of southern Velen (east of Reardon Manor). He has contracted the plague and is begging for someone to take his life. You can choose to grant him his wish, if you want.
