Yes PS Plus will indeed be required to play Diablo 4 on PlayStation. The game’s page over on the store lays it out pretty clearly, stating that PS Plus is necessary for online play and that online play is also a requirement. That’s about as crystal as it gets. If you need further evidence, then check out this post from Reddit user WordCoding. Leaving the grating in-character way the support person phrases their response, they say that “each player needs their own PSN sign in which inturn (sic) needs its own subscription.” This means that every person you want to team up with will have to have their own PS Plus ready to go, even if you just want to enjoy Diablo IV in couch co-op (assuming that’s even an option).

So, yeah, that’s all there is to know on the subject of whether PS Plus is required to play Diablo 4 on PlayStation. I know that a lot of people are not going to be overly enthused about this, but it is what it is. Personally, I’m not a huge fan of games that force me to always be online, even if I just want to play single player. That said, if you have multiple accounts on one PlayStation, it might be possible to finagle things so that only the primary account requires a PS Plus subscription, so long as there’s just one player online at a time. I don’t know, we’ll have to wait and see. Best have an account paid for just in case.
