The important thing to keep in mind is that the new gear is available only in New Game Plus. It was added as a way to let your character get stronger in the face of more powerful enemies. It’s not available for characters on regular saves, no matter the difficulty you chose. The developers didn’t seem to think it would be necessary to have them. This means you’ll have to transfer your save and start New Game Plus if you want the new weapons and armor.

As for actually acquiring them once you’re in NG+, it’s pretty much the same as with all the others. You’ll need to buy them from a merchant. Meridian is your best bet, since the traders there are well stocked and all in the same spot. You’ll need to do a bit of shard farming, though, as they’re far from cheap.

On the other hand, you’re not really missing out on anything. The new items aren’t really new, they’re only upgraded versions of existing stuff. They look the same, they act the same and they have the same stats. The only difference is that they also have an additional mod slot, which brings the total to four. So yeah, they will make you stronger – a lot stronger if you choose the correct mod combos – but you won’t experience anything new.
